There is a link to the form you need to fill in. It won’t cost you anything at all. Some people are charging just to let you have the form. (Some are also, more importantly, getting people to divulge their Social Seurity numbers, and they are nothing to do with the Social Seucrity Administration, so they are not entitled to it).
